Charles "Chucky" Blake

Charles “Chucky” Blake 37, passed away Saturday, September 12, 2015 at Mount Carmel East Medical Center in Columbus.

Chuck was born in Zanesville on October 16, 1977. The son of Wendy (Gottke) Blake of Lockbourne and Steven Blake of Duncan Falls. He lived in Gahanna where he worked as a Truck Driver for Columbus Distributing; He was a 1996 graduate of Crooksville High School and a member of the Teamsters Local #284.

Chuck is survived by his wife, Jamie Blake whom he married December 13, 2014; his father, Steven (Becky) Blake of Duncan Falls; his mother, Wendy Blake of Lockboune; his daughter, Mattison Melvin and his children, David, Isabella and Caitlyn; his brother, Steven L Blake (Kristen) of Zanesville; and a host of aunts, uncles, cousins and friends who loved him very much, including a special friend, Jack Carnes.
Chuck is preceded in death by his maternal grandparents, Lloyd Gottke and Joan Murphy; his paternal grandparents, Herbert and Barbara Blake; his uncle, Dan Blake; and his 3 legged best friend, Jake who passed away a year ago.
Friends and family may call from 2 to 4 and 5 to 8 pm Thursday, September 17, 2015 at the Farus Funeral Home of Duncan Falls where funeral services will be held 1 pm Friday.
The family encourages his friends to come dressed in “Chuck causal” attire.  The family would like to thank everyone for their outpouring of love during this difficult time.
As much as Chuck gave through life, he also gave through death by giving the gift of life with organ and tissue donation.
